Job Description:

patients 60 and up only in medically under-served metro areas, M-F,
no nights, weekends, or call and an average of 15 patients per
day.
Above-market compensation. Incentives based on quality metrics.
Annual bonus of up to 20% of salary. $320k+ yearly earnings
